Abstract

Creation of ED`s with their own ICU may enhance the deficiency of anesthesiologists which have existed in Russian Federation. The article is about of comparison of effectiveness in that units anesthesiologists versus emergency physicians with additional training. Thereby, a possible way to solve the personnel problem is the optimization of EP`s post-education program.

Abstract

For optimization Krasnodar region medical aid service, it’s developed software global navigational control system. Medical aid team is supported with pads. The research aim: estimate of efficiency and supporting medical aid team mobile computer devices with remote access to “AWP manager” program under Krasnodar region common control room medical aid service. The development of common informational control system of medical aid and medical aid team supporting with pads are improved basic processing indices of its work: time for medical aid teams arrival is reduced, including TA; execution of the call; TA death rate indices are reduced. The development makes it possible to carry out reception, collection of information and data handling automation about the service work and standardization, sorting calls according importance and complexity, controlling the time and the quality of medical aid service and hospitalization.

Abstract

The aim of this study was to investigate the efficiency of National Service of Medical Aviation (St. Petersburg, Russia). This is anesthesiologist-staffed helicopter emergency medical service (HEMS) mostly intended for the from-the-field and interhospital transport of patients with severe trauma and life-threatening diseases from rural hospitals to specialized medical centers in St. Petersburg. In total, 219 patients were included, that were transported by HEMS in 2017. 44,3% of patients had major or multiple injuries, 21,0% suffered from acute heart diseases, 15,5% had strokes or brain trauma. No one patient’s death during the flight was observed. This study findings provided valuable information that may have an impact on the current staffing and accreditation criteria for HEMS operations in Russia.

Abstract

The aim of the study was to study the results of treatment in primary vascular center of patients with suspected acute coronary syndrome (ACS). The study included 543 patients with suspected ACS, delivered by emergency medical service or who applied consistently for 3 months to the primary vascular center and 10 patients with myocardial infarction, hospitalized initially with other diagnoses. The results of electrocardiographic study, studies of markers of myocardial necrosis, coronary angiography, the proportion of hospitalization, 30-day outcomes and cases of repeated inpatient contact were evaluated. Low diagnostic capabilities of the hospital’s receiving department, which does not have an emergency department, in relation to the risk assessment of the patient with chest pain were revealed. Low possibilities of hospitalization of patients with suspected ACS have been established, which leads to delayed hospitalization and mortality of non-hospitalized patients at home. Errors in the diagnosis of myocardial infarction at the prehospital stage lead to diagnostic errors in the hospital. More frequent use of qualitative evaluation of myocardial necrosis markers at the prehospital stage is suggested as ways of solving the problems.

Abstract

62 patients were admitted to Alexandrovsky City Hospital with acute severe ethanol intoxication (coma 2). The first gr. (21 pts) received by IV bolus the combination of Molixan (3,0 мg/kg) and Cytoflavin (0,14 mg/ kg); 2nd gr (21 pts) received Molixan only (3,0 мг/кг IV bolus). The interventions were identical in all groups including controls (20 pts) via standard therapy (dextrose, saline, vitamins). The awakening effect was achieved in 3,0± 0,1h (molixan+cytoflavin), and 11,5±1,2 h (controls). Molixan and Cytoflavin administration also reduced lactate acidosis, increased quality assurance, and length of stay in the Hospital, as such.

Abstract

The aim of the study was to identify among the injured in road accidents in the Lipetsk and Voronezh regions the category of persons whose lives could potentially be saved by timely first aid. The subject of a study were «Acts of the forensic medical examination of the corpse» and «The card of the injured in a traffic accident with the list of the realized first aid measures». According to the study 23,4% of injured had a chance to survive in case of timely first aid provision. This finding demonstrates the necessity of first aid provided on the scene of the road crash.

Bleeding after tonsillectomy is a serious complication that requires emergency care. The tactic of treatment for bleeding after tonsillectomy depends on the intensity of bleeding and predisposing factors. The paper presents an analysis of the causes of secondary bleeding in patients after tonsillectomy.

Abstract

In work terms of emergence and frequency of development of a hypothermia in patients with traumatic shock of varying severity at a prehospital stage are presented. It is revealed that all patients with traumatic shock have III severity, unlike patients with shock of I and II severity, decrease in body temperature is noted.

In article results of the retrospective analysis of clinical records of the teenage girls hospitalized in the emergency order with uterine bleedings are described. In group of patients with the accompanying somatic pathology the forecast of a course of a disease and medical tactics has reliable distinctions in comparison with group of patients with not burdened anamnesis on somatic pathology.

Abstract

The article presents the results of treatment of 260 patients with gastric ulcer complicated by hemorrhage, who were on treatment in the 2nd clinic (surgery for the improvement of doctors) of the Military Medical Academy on the basis of the Institute of Ambulance im. Dzhanelidze in 2014–2015. The obtained data indicate that the reduction in mortality in the study groups can be achieved by: reducing the number of palliative interventions, the wider use of angiographic embolization for the prevention and treatment of bleeding recurrence, and expanding the indications for urgent radical surgery outside the recurrence of hemorrhage.

Abstract

The article presents a methodology for planning the differentiated volumes of emergency medical care based on statistical data on the indicators of population circulation. An algorithm for monitoring the basic indicators of the activities of emergency departments in the Excel format was developed, which determines the organizational structure of emergency medical care at the level of the municipal formation.
