Abstract
Results of studying of opinion of the patient on quality and availability of an emergency medical service in St. Petersburg are presented in article. 450 patients brought to the St. Petersburg Scientific Research Institute of Emergency Medicine of I. I. Dzhanelidze by crewsof an emergency medical service in June, 2015 were for this purpose interrogated. Results revealed an appreciation of work of an emergency medical service of St. Petersburg, a significant role of satisfaction of patients in an assessment of availability and quality of an emergency medical service, an important role of the dispatcher of an emergency medical service in evaluation of the work of an emergency medical service in general.

Abstract
A comparison of pre-hospital t and hospital thrombolytic therapy (TLT) on the clinical and economic indicators at patients with myocardial infarction (MI) was estimated. It is found that the pre-hospital thrombolytic therapy compared with hospital reduced duration of hospitalizationin patients with MI. Economic losses from premature mortality at the hospital thrombolysis exceed similar losses at a pre-hospital trombolizis twice. A time “symptom - needle” decreases for one hour.

Abstract
The problem of early diagnosis of acute infectious diseases with exanthemas represent a big challenge for the paramedic medical advisory service. So we studied causes of administering emergency medical care by the ambulance crew of infectiologists in Moscow’s hospitals. We determined the role and the diagnostic importance of the ambulance crew in early prehospital diagnosis of infectious diseases.

Abstract
On the basis of the analysis of 131 ambulance medical cards (7 cities of the Russian Federation) it is established that intensity of acute pain of nonspecific character according VAS in a low back at pre-hospital stage exactly doesn’t lead to changes of arterial blood pressure and cardiac rate and doesn’t depend on nerve root involvement symptom, on age and on gender. Using of a ketoralak tromethamine, ketoprofen and SCENAR-therapy for pain relief is comparable on analgesic effect. Main differences in using between preparations and treatment methods are in the sphere of safety and comfort of using. A basis of a pain relief method choice is absence of contraindications for preparations or a method.

Abstract
Treatment of 400 patients with acute decompensation of essential hypertension (EH) was analyzed according to emergency call registers in four Russian first-aid stations retrospectively. First-aid station team rarely follows the clinical guidelines for management of patients with acute EH exacerbations, the diagnosis and treatment of disease destabilization causes aren’t satisfactory.

Abstract
The paper presents the analysis of nosological entityntity structure of patients being hospitalized and treated at the short term treatment department of I. I. Dzhanelidze Research Institute of Emergency Medicine for the period of a year 2012. From 3037 patients admitted mostly from emergency department (97,21%), there were 29,96% of patients admitted from the yellow zone, 1,09% of cases from red zone, 67,86% of cases from green zone. It has been found out that 58,90% of patients were hospitalized for the short term treatment, 37,44% of patients were hospitalized for making a diagnosis. Abstract
Diagnosis of acute pyelonephritis in accordance with the concept of the role of the immune system in the development and further exacerbation of the inflammatory process is an urgent task. The purpose and objectives of the study was to determine the severity of acute pyelonephritis by examining changes in the level of pro-inflammatory and anti-inflammatory cytokines. The results of the study of proinflammatory cytokines and anti-inflammatory in the urine taken from the ureter, bladder and blood plasma to determine the severity of an exacerbation of chronic pyelonephritis. Proved highly informative IL-8 levels in the urine from the ureter and ureteral catheterization safety during exacerbation of chronic pyelonephritis, which made it possible to carry out a differentiated treatment depending on the severity of the inflammatory process, improve outcomes and reduce the duration of hospital stay.

Abstract
Studied the results of primary total hip arthroplasty in 62 patients admitted in Emergency Hospital Medical Services with various types of pertrochanteric fractures by classification AO-ASIF. All patients were set foot prosthesis cemented (cement type legs Muller). Concluded that hip arthroplasty with cement- retained prosthesis stem may be the method of choice in stable pertrochanteric fractures of type A1 and stenting in unstable fractures type A2 requires further study.
