Оptimization of the reason for the call «Heart condition (pain behind the sternum)» for NSR teams: characteristics of measures and evaluation of the results of implementation

Abstract

The purpose of the study was to analyze the database and report forms of call cards with the reason «Heart trouble (chest pain)» and with the diagnosis code «I21» according to ICD-10 in the Emergency Management System program for the period 2019–23, to develop and implement optimization measures and evaluate the results implementations for the period 16.03.24–15.03.25

The materials of the database and reporting forms of call maps of the NSR station for two periods were used: 2019–23 (before the implementation of optimization measures) and 16.03.24–15.03.25 (after implementation), analytical and statistical methods were applied.

The results of the study. When providing emergency medical care to patients with acute myocardial infarction, the proportion of clinical deaths in general medical teams decreased by 4 times (from 0.8% to 0.2%) and by 3 times (from 3.8% to 1.2%) in anesthesiology and intensive care teams relative to the initial study period. In 2024, relative to 2023. The number of patients in severe and moderate severity diagnosed with acute myocardial infarction transferred to the anesthesiology and intensive care teams decreased by 2 times due to a twofold increase in the proportion of calls made by anesthesiology and intensive care teams to patients with acute myocardial infarction (from 14.9% to 28.7%) after the introduction of an optimized reason the challenge.

Conclusion. The creation and introduction of the reason for the call «A man over 40 years old with a bad heart (pain behind the sternum)», which is specialized for anesthesiology and intensive care teams, made it possible to increase the availability and improve the quality of NSR care for patients with acute coronary syndrome with ST segment elevation, with other diagnoses of category «I» according to ICD-10, requiring participation of anesthesiology and intensive care teams (from 26.8% to 38.7%). This reason for the call is recommended for implementation in the work of NSR stations, which include teams of anesthesiology and intensive care.
