Closed traumatic skin detachment: development of a modern method for early diagnosis at the stage of emergency medical care
Abstract
Each case of traumatic skin detachment has its own characteristics, taking into account the localization and severity of the lesions. To date, there are no clinical recommendations and standards for the treatment of patients with this pathology approved by the Ministry of Health of the Russian Federation. The scope of examinations, treatment methods, and criteria for its effectiveness in such injuries are very controversial. At the prehospital stage, patients with closed skin detachment are often diagnosed with soft tissue contusion and subcutaneous hematoma of the injured area, which leads to diagnostic errors and complications in early or long-term periods in the form of necrosis of the skin of the injured area. Since the diagnostic methods used in surgical practice are invasive or require the availability of high-tech equipment, their use at the pre-hospital stage of care or injury level 1 and 2 centers are not possible. One of the objective methods for diagnosing closed traumatic skin detachment at the prehospital stage may be laser Doppler flowmetry. The values of the perfusion index corresponding to closed traumatic skin detachment were determined. The diagnostic criteria for making this diagnosis were determined, which allowed us to propose an algorithm for diagnosing closed traumatic skin detachment. The application of the approaches developed by us to the diagnosis of closed traumatic skin detachment will improve the results of treatment of this category of victims.
