This article presents the results of analysis of control activities of the Federal Service for Surveillance in healthcare (Roszdravnadzor) on the organization of emergency medical care inRussian Federationand determined the main typical breach. Discusses recent changes in the legislation in terms of improving the organization of emergency medical care system inRussian Federation. Suggests key areas for further improvement of emergency medical care.
In light of introduction of high-tech diagnostic and treatment methods into medical practice the delivery of information on the achievements of science and technology to the final user— the practitioner is becoming more and more actual target. The development of information technologies has made it possible to use them for this purpose. Unfortunately, today there is a big different between the capabilities of medical professionals living in cities and their colleagues from the provinces in questions of new medical information. Main idea and purpose of this work is an attempt to analyze how to use the capabilities of modern telecommunication technologies to bring to the ambulance practitioners actual high-tech medical information and also using these capabilities to estimate the level of their professional skills.
91104 calls were analyzed for the period of 10 years, the occurrence of hypertension increased from 324,3 to 423,8 cases for 10000 inhabitance/year. The majority of calls were multiple and undertaken to the same patients suffered from moderate, not life-threatening hypertension. To decrease the number of such callswe considered the possibility to implement self-care programs for these group of patients. Prehospital advanced treatments included moxodonine administration, and its combination with furosemide, or nifedipine, gives the possibility to develop the treatment plan exactly “on the field”, with the evidence of safety and efficacy for health care provider and his patient.
This article presents the results of a retrospective research study on the outcomes of chest compressions through the traditional “manual” method and with the use of the electromechanical device LUCAS-2. The collected data did not provide us with evidence of a significant discrepancy in the results of two compared groups of patients; immediate that in certain situations encountered byEMS, it is not always possible to provide a patient with manual chest compressions, but conditions could make it possible to continue with the use of devices such as the “LUCAS-2”. These circumstances make it possible to recommend a wider usage of electromechanical devices such as the “LUCAS-2”in the EMS practice inSt. Petersburg.
This study demonstrates the effectiveness of propafenone for pharmacological cardioversion in patients with paroxysmal atrial fibrillation. Propafenone was used in the recommended dosage 2 mg/kg. The restoration of the sinus rhythm within 60 minutes was observed in 73,2% of cases.

The article highlights the burden of the venous thromboembolism (VTE) in clinical practice as one of the most frequent cause of cardiovascular death and disabling embolic stroke worldwide. The author compares the traditional use of VKA antagonists and new class of oral anticoagulants— direct thrombin and factor Xa inhibitors (dabigatran, rivaroxaban, apixaban) for VTE treatment and prevention. The results of most significant randomized clinical trials in VTE area and the latest international guidelines are also discussed.
Allergy is an adverse immune system reaction to a particular allergen. In recent years there has been an increase in the number of allergic diseases, the most severe of which is anaphylactic shock. Our review reflects the international experience of the strategy of treating anaphylactic shock and is aimed at determining the correct tactics of managing a patient with this severe pathology.

INTRAHOSPITAL EMERGENCY MEDICAL CARE
Introduction of an emergency department hospital to practice of a multi-disciplinary hospital allowed to increase the proportion of completed cases among them the most evident are the cases of the following ICD-10: «III»— in 10.95 times, and «XI»— 9,89 times, «IX»— in 6.19 times, «X»— in 6.15 times, «XIV»— in 5.96 time, «II»— in 5.35 times, «XIII»— 5.01 times. The analysis of diagnoses, markers of unreasonable hospitalization (K29, K86, N11)— in 13.3 times (p<0.05). This was achieved by means of quality Improvement activities (reduction of frequency of differences in diagnoses by 1.6 times from 24.6 to 15.4 cases per 100 cases) and of availability (reducing the waiting time of initial examination by1.93 times from 17.1 to 8.9 min).
Clinical efficacy of Molixan in acute severe poisoning by corvalol is presented in a randomized placebo control study. 20 patients with severe acute poisoning by corvalol were inrolled into 2 groups. The interventions were identical except the observation group received Molixan was included in intense therapy. To compared recovery of consciousness, cognitive function, serum concentration of toxicant, simple visual-motor reaction time and complex visual-motor reaction time of discernment. It was demonstrated that the use of integrated drug molixan (regulator of energy metabolism and ligand of GABAA-receptors) was provided faster and more qualitative recovery of consciousness, cognitive function, reduced the duration of asthenic period in complex intensive therapy of toxic and hypoxic brain damage in acute severe poisoning by corvalol.
