Abstract

Approval of the Order of Ministry of Health Care and Social Development of the Russian Federation 17.05.2012 № 555n allowed to initiate the process of formation of bedspace in emergency department of the hospital (daily beds and beds of short stay unit). 2012 through 2016 patient capacity in the bed space of the Russian hospitals is increased by 122 to 530, the rate of hospital beds is increased by 0,01% to 0,05% (among them the rate of hospital beds of short stay units is 64,7%, daily beds — 35,3%). Average annual bed occupancy rate is reduced by 343,1 to 208,3, yearly bed turnover increased by 68,6 to 355,9, death rate reduced by 1,21% to 0,17%. The results indicate the establishment of a new organizational technology management in emergency departments of multidisciplinary hospitals in Russia.

Abstract

2012 through 2016 patient capacity in the bed space in hospitals of Saint-Petersburg is increased in number of 40 to 144 or 0,12% to 0,41% in share (among them the rate of hospital beds of short stay units is increased to 47,1%, rate of daily beds is increased to 52,9%). Ratio of number of patients referred from emergency unit to specialized units is reduced by 26,1% to 14,5%. Average annual bed occupancy rate is increased by 435,8 to 167,7. Hospital bed turnover is increased by 64,4 to 201,6. The death rate is reduced by 0,41% to 0. Bed population ratio in Saint-Petersburg (27,3 per 1 million of population) is below the recommended level (111 beds for 1 million of population).

Abstract

Article is educated to opportunities of a magnetic and resonant and computer tomography in diagnostics of cord compression in patients with trauma of the cervical spine. Described semiotics of spinal cord compression, the technique of the quantitative assessment of canal stenosis and spinal cord compression. The analysis of bone injury with the release of the type of damage, determining the amount of damaged columns of the cervical vertebra. The contribution of damaged spinal elements to compression of the spinal cord is analyzed with the definition of the main compressing factor, which allows choosing the most optimal type of treatment.

Abstract

The paper displays the need for an extended electrocardiographic diagnosis including right breast leads for all patients with lower myocardial infarction at early stages of medical care. The diagnosis method presented here will allow to evaluate the localization of myocardial damage and to identify patients with signs of right ventricular damage that require clinical evaluation to identify signs of right ventricular failure and special methods for its correction.

Abstract

The condition of medical service organization by the patients with the acute coronary syndrome (ACS) in the ChuvashRepublicis presented. Pharmacological and invasive methods of treatment for this time go around to the fullest extent. Thrombolytic therapy, especially at a pre-hospital stage, is the main reperfusion strategy in case of acute myocardial infarction with raising of ST segment (AMISrST). In theChuvashRepublic the system of common approach of assistance in case of acute coronary syndrome with use of innovative approaches in the choice of modern thrombolitycs is developed.

In work the optimized program of infusional therapy applied at patients with hemorrhagic shock of varying severity at pre-hospital and hospital stages of treatment is submitted. It is taped that this program of infusional therapy effectively korrigirut disturbances of a systemic hemodynamics and doesn’t make impact on a hemostasis and electrolytic structure.
