Abstract

In article  historical materials on creation of scientifically research institute of ambulance in St. Petersburg, the  main  milestones of its  development are  briefly  stated, the  contribution of institute in  improvement of system  of rendering emergency and  emergency medical service  in the  country is shown. For the  nice 85-year history  Institute  I made the  significant contribution to  studying and  the  solution of scientific  and practical and  organizational and  methodical questions in the  field  of an  emergency medical service  in St. Petersburg and  in the Russian  Federation.

Abstract

We have  analyzed the treatment of patients with  acute  non-traumatic spinal  disorders in the urban  center of spine  emergency surgery.  It has  been found, that  such  organizational solution provides good  immediate results  and  maintains them  during  a late postoperative period.

Abstract

The analysis  of the organization of mental health services  to patients of a multi-profile hospital from 2009 for 2015 is provided. The  cliniсo-epidemiological analysis  of prevalence,  structure of mental pathology among the  patients hospitalized in somatopsychiatric department and  the  center of treatment of sharp  poisonings  of a multi-profile hospital is carried out.  The annual average percent of people who  need the mental health services  constituted 3.39±0.16%  of a total  quantity of the patients and  victims  who  are on treatment. The efficiency of use  of bed  fund  of somatopsychiatric department for the  long-term period is shown. The considerable prevalence of patients with  the  organic  mental disturbances, frustration connected with  acceptance  of psychoactive agents, neurotic and  somatoform disorders in general structure of mental pathology is established.

Abstract

Results  of surgical  treatment of gaps  (at 3 victims)  and  chipped wounds (at 2 people) a chest  aorta  are analyzed. To four of them  bleeding was  stopped in the  traditional way  (a torakotomiya, a sternotomiya), to the fifth — by stenting of a vessel.  2 patients with the closed breast  injury died, the 3rd survived.

Abstract

The research of 28  women of reproductive age  after  a laparoscopic resection of the  only  ovary  about the complications of benign tumor  and  ovarian  formations. It is shown that  electrothermic coagulation makes  more rough  impact on ovary tissue,  causing destruction of the follicular  device. In all respects researches (menstrual and reproductive functions, hormonal markers of an ovarialny reserve) the acceptable method of a hemostasis is-ligature.

Abstract

The  results  of the  treatment of 182  patients with  pancreatic trauma are  presented. The  use  of an  advanced diagnostic and  treatment algorithm, included the  routine use  of modern instrumental diagnostic methods, such  as ultrasound, computed tomography, diagnostic laparoscopy, limitation of indications for plugging, omentobursostomy, suturing  capsule of pancreas, cholecystostomy, has reduced the  time  elapsing from the  moment of hospitalization to operation an average from 9.3  to 3.3  hours,  to reduce the  frequency of septic  complications from 16.5% to 8.2%, and  the percentage of mortality from 24.7% to 18.8%.

Abstract

We analyzed the results  of treatment of 142  patients with  tumoral origin stomach bleeding. In the control group  there  was  no  unified  therapeutic conception and  patients were  operated mainly  on  an  emergency basis  at admission or in case  of recurrent bleeding. In the  treatment of patients of the  main  group  was  used individual medical-diagnostic tactics  taking  into  account the  degree of endoscopic hemostasis, the  severity of the  bleeding and  the  patient’s condition.  This approach allowed to  decrease the  rate  of postsurgery complications of patients with  tumoral origin  stomach bleeding from  45.8% (22)  to  22.5% (9) and  the mortality from 35.4% (17) to 5.0% (2) (p<0,001).

Abstract

Different  issues  regarding the  arrangement of urgent  medical care  to the  patients with  embolic events of aorta  and  main  arteries  of limbs  in Saint-Petersburg for the past  25 years were  studied. There was performed a comparative analysis  of outcomes 863  patientswith acute  limb  ischemia treated in specialized vascular surgery  department and  897  patients treated ingeneral surgery  departments. The  superiority of  urgent surgical  revascularization under  conditions of specialized department was demonstrated.

Abstract

Spice  overdose became a leading type  of poisonings among illicit  designer drugs.  The particularities of natural  and  synthetic spices  with  the  hallucinogenic action are  discussed with  determination of specificity in  adrenergic  toxsyndrome.  Administration of  cholinergic and  adenosinic  medications ameliorate the consequences of psychosis and  adrenergic toxsyndrome, and  seemed to be the  novel  approach in treatment of spice  poisonings.

Abstract

Currently, the  pathogenesis of widespread peritonitis (RP) attaches great  importance to  increase intraabdominal pressure (IAP) at  which   there  is  a  substantial  violation of  the  circulation of  the  abdominal cavity  and  increases endogenous intoxication (EI). According to  the  dynamics of intra-abdominal pressure can  be  seen  on  the  course of the  pathological process in the  abdominal cavity.  In work  the  assessment of the  diagnostic significance of increasing intra-abdominal pressure in the  progression of peritonitis and  the development of complications in the postoperative period.
