Abstract

This article presents the quality assessment criteria for emergency care and specialized emergency medical aid at hospital admission. 206 disease groups approved by Order of the Ministry of public health of Russia from 10.05.2017 № 203-n are described in the article. The article deals with fulfillment of the requirements of the order № 203-n is possible only by changing the model of the admission unit to the model of inpatient emergency department.

Abstract

In the light of informatization of health care of the Russian Federation 2018 became a new stage of development of telemedical technologies in our country. The assessment of a current state of telemedical services in the Russian Federation was a research objective. The content analysis, legal, analytical methods are used. The telemedicine allows to make the state health care more effective and cheaper, considerably to optimize productions and logistics of the medical organizations, to provide increase in volumes of the rendered medical services. It is especially important within an emergency medical service when the distance and time are critical factors of assistance.

Abstract

The workload on the emergency medical services due to patients’ arterial hypertension calls has not been studied enough. Both the number of emergency calls from urban residents of the Russian Federation suffering from hypertension and the dynamics of the calls from 2008 to 2017 have been evaluated.

Abstract

The analysis of the peculiarities of rendering psychiatric care to patients with alcoholism and harmful alcohol use in a multidisciplinary hospital for the period from 2009  to 2016 is presented. with the establishment of multi-year trends, dynamics and structure of morbidity. The average annual and average daily intake of patients with an alcoholic factor was analyzed. It is established that 20.1% of those entering the multidisciplinary hospital suffer from alcoholism and the harmful use of alcohol. The prevalence of residual psychotic disorders and alcohol withdrawal with delirium in the structure of alcohol pathology in patients with somatopsychiatric department is proved. The predominance of combined somatic pathology in patients suffering from alcoholism, weighting the course of the mental illness, is established.

Abstract

A retrospective content analysis of 210 emergency medical call cards with paroxysms of atrial fibrillation up to 48 hours was carried out in order to analyze and improve the algorithm of prehospital therapy of emergency arrhythmias based on the study of the peculiarities of amiodarone action in elderly and young patients in emergency care.

Abstract

The article presents the experience of using an improved model of the trauma registry. The model allows you to register all the injuries to the victim in accordance with the International Classification of Diseases ICD-10 and the Abbreviated Injury Scale severity index. The results of monitoring the structure of multiple and combined injuries in 548 patients are presented.

Abstract

For the purpose of justification for the use of various minimally invasive interventions in the treatment of patients with moderate to severe acute pancreatitis, the authors retrospectively analyzed the data of medical documentation for the period from 2009 to 2017, shows the results of surgical and medicinal treatment, the structure of mortality and complications depending on the choice of surgical manual for patients with acute pancreatitis. The possibilities of minimally invasive methods of surgical treatment in comparison with traditional operations are presented. It is shown that the use of minimally invasive techniques in the treatment of pancreatic necrosis significantly reduces the level of mortality associated with complications of infected pancreatonecrosis, and also reduces the level of postoperative complications.

Abstract

In work are presented results of the research executed at 285 patients with traumatic shock of varying severity at which defined development of the acute respiratory distress syndrome (ARDS). It is taped that at patients with traumatic shock of the I degree ARDS phenomena don’t become perceptible. At patients with traumatic shock of the II degree emergence of ARDS in 17,8%. At patients with traumatic shock of the III degree emergence of ARDS in 100%.

Abstract

There were analyzed results of treatment of 246 patients with purulent cholangitis and obstructive jaundice with designing and implementation of original algorithm for management in an emergency hospital with usage of minimally invasive interventions and sepsis treatment. The article is illustrated with self-findings.

Abstract

In 2015–2016 the research on use of a terlipressin at a pre-hospital stage of emergency medical service in BU RSSMP of Chuvashia was conducted. At bleeding presumably from VRV PZh at a pre-hospital stage entered i.v. bolyusno a synthetic analog of a vasopressin Terlipressin in a dose of 1–2 mg to 310 patients, at the same time results of treatment of 236 cases at which bleeding from VRV PZh was confirmed further in republic hospitals are analysed. Results of early intravenous administration of drug Terlipressin allow to judge expediency and efficiency of its use at a pre-hospital stage, before endoscopic specification of a bleeding point.

Abstract

Acute urinary retention is a condition characterized by a sudden inability to urinate, which is accompanied by severe pain and an intense urge to urinate. The article discusses the predisposing factors for this pathological condition, the role of combined drug therapy as a method of its prevention, assessed modern methods of drainage of the lower urinary tract during acute urinary retention.
